CLINICAL CASE OF TEMPOROMANDIBULAR DISORDER

SUMMARY OF PATIENT HISTORY

About a year ago it started with several episodes of TMJ blockage, more to the right. The blocks were more intense during chewing and associated with pain. Drilling was recommended by dentistry but the patient was unable to tolerate this. Feels a worsening of the clicks. Denies limitation of mouth opening and denies difficulty in chewing.

MAIN COMPLAINTS

> TMJ clicks

> TMJ blocks

> Right TMJ pain 5/10 VAS

> Left TMJ pain 4/10 VAS

SUMMARY OF THE OBJECTIVE EXAMINATION

> 43mm AMO

> Right TMJ: G3 Synovitis (pain on mouth opening) with suspected disc displacement without reduction

> Left TMJ: Synovitis G1 with suspected disc displacement with reduction

> Grade 3 muscle contracture

DIAGNOSIS

RIGHT TMJ:

> PREVIOUS DISPLACEMENT OF DISC WITH NO REDUCTION WITH ARTICULAR SPILL

LEFT TMJ:

> DISPLACEMENT WITH REDUCTION WITH ARTICULAR SPILL

> GARDE 2 MUSCLE CONTRACTURE

TREATMENT

> BILATERAL OPEN DISC REPOSITIONING SURGERY

> BOTOX PROTOCOL

> STOMATOGNATHIC SYSTEM REHABILITATION
